How does it work?

  1. Go to www.eenhuisartsdichtbij.nl and click on ‘Find a GP near you’.
  2. Enter your postcode and search for a GP in your area.
  3. Is a GP practice open to new patients? Then you can click straight through to the registration form.
  4. Follow the steps in the form to submit a registration request to do.
  5. Your application will be assessed. You will be notified as soon as possible whether you can be enrolled.

A GP Nearby is for people who do not yet have a GP. Do you already have a GP in the region? Then you cannot request a transfer via this route. If you do submit a request, we will see that you are already registered and cannot process your request.

What happens after I fill in the registration form?
Nadat u het inschrijfformulier heeft ingevuld en verzonden, ontvangt onze praktijk uw gegevens. Wij bekijken vervolgens of inschrijving op dit moment mogelijk is. U krijgt zo spoedig mogelijk bericht van ons om te laten weten of wij u kunnen inschrijven.
Your registration is pas definitief zodra wij dit hebben bevestigd.

Do I have to inform my current GP myself when switching?

When you register with our practice, you give us permission to deregister you from your current GP.

However, we do advise you to also inform your current GP yourself that you are switching.

Kan ik eerst kennismaken voordat ik mij inschrijf?

U mag uiteraard ook eerst langskomen voor een kennismaking. Neem telefonisch contact op met onze assistente om een afspraak te maken.

What if I am already registered and my partner or child also wants to become a patient?

A practice may be temporarily closed for new registrations. However, are you already registered with us and your partner moves to the same address? Or are you having a child? If so, we always try to make room within the practice in these situations.
