Patient environment Your Care Online

With the patient portal Your Care Online makes it easy to take care of your health matters, at your convenience. There is no need to call or drop by. The portal is available 24/7. You can:

  • Requesting repeat prescriptions
  • Submit a medical question to your GP, pharmacy or practice manager (e-consultation) 
  • Making appointments with your GP
  • Viewing your medical records, such as hypersensitivities and allergies, test results and letters and referrals
  • Viewing your current medication overview

Your data is well secured and only you and your healthcare providers have access to your record.

Frequently asked questions

How do I log in to Your Care Online?

To log in to Your Care Online, follow these steps:

  1. Go to the Your Healthcare Online website or open the app.
  2. Click on 'Login'.
  3. Choose whether to log in with DigiD or with your e-mail address and password.
  4. Follow the on-screen instructions to enter your details.

If you experience problems logging in, please check that your e-mail address is entered correctly and that your password is correct. If you have persistent problems, please contact the practice or the Your Care Online helpdesk.

How do I create a Your Care Online account?

If you are registered as a patient in one of our GP practices and are 16 years or older, you can use this service. To create an account, please follow the following steps:

  1. Go to Your Care Online and find your healthcare provider.
  2. Click on the patient environment link or on Login.
  3. Select 'Create a new account' and follow the steps on the screen.
  4. You will receive an e-mail with an activation link. This e-mail may be in your junk mailbox. Click on it to activate your account.
  5. Log in with your e-mail address and password. Enter the security code from the e-mail.
  6. Click on Add care provider and find your practice.
  7. Wait for confirmation from your healthcare provider to link the account (this may take a few days).

What should I do if I forget my password?

If you forget your password, you can reset it in the app and via your healthcare provider's website. You do so by doing the following.

  1. Click on 'Login'.
  2. Under the fields for e-mail address and password, select 'Forgot password?'.
  3. Enter your e-mail address and click on 'Send code'.
  4. Open your e-mail and note the received verification code.
  5. Enter the code on the screen and click on 'Verify'.
  6. Choose a new password and click on 'Save password'.
  7. You will return to the login screen and can now log in with your new password.
