Looking for a GP in Amstelveen?

You are welcome to register with us as a patient, provided you are not already registered with another GP in Amstelveen and live within our postal code area (1187). When you visit us for the first time, we kindly ask you to bring a valid proof of identity (passport or ID card) and your insurance details.

Note: Please note that completing and submitting the registration form does not automatically mean you are enrolled; once we receive your form, we will contact you as soon as possible to let you know if we can enroll you.

Sharing your medical data

Our GP practice uses Mitz, a nationwide, secure and user-friendly online environment. In it, you decide which healthcare providers are allowed to view and share your medical data, such as the pharmacy, GP out-of-hours surgery or medical specialists. This can be very important to help you properly and quickly. Your data will only be shared if you give your explicit consent. Therefore, in addition to completing the form, we ask you to arrange your consents through Mitz.

Access to the patient portal

Once your registration has been confirmed, you can create an account via the Uw Zorg Online website. This will give you access to the patient portal. In the patient environment click on ‘Create a new account’ and follow the four steps that appear on the screen. Once completed, you will receive an e-mail with an activation link. Clicking on this link will activate your account and allow you to log in to the patient portal.
